1. - the incident is reported

2. - investigate the accident
For each (sub)category, decide who will be automatically notified should be when an accident is reported. In the case of a near accident, management may not want to be notified of this, but in the case of a fatal accident, they do.
Possibly you can direct actions Take steps to secure the environment. Use standard tasks to be reminded of things you must always deal with in that type of incident (reporting to insurance, making circumstantial report, accident report card...).
Only employees with the appropriate authority (at least prevention advisor level 2) can be designated to perform the root cause analysis perform.
